Public Notice of Health Care Facility Closure

Effective on or about 12:01 AM on June 1, 2023 (the “Closure Date”), St. Louis Cardiovascular Institute, LLC (the “Company”) ceased operating St. Louis Cardiovascular Institute, LLC d/b/a St. Louis Cardiovascular Institute, located at 1028 S. Kirkwood Rd., Suite A, Kirkwood, MO 63122. As of the Closure Date, the Company no longer participates in the Medicare Program (Title XVIII of the Social Security Act), and the related agreements with the Secretary of the United States Department of Health and Human Services will be terminated in accordance with the provisions of the Social Security Act.

The Medicare Program will not make payments for services furnished to patients on or after June 1, 2023. If anyone has questions regarding this notice, please call 314-394-2950.
